Head and neck squamous cell tumours with extracapsular extension (ECE) are known to have a poorer prognosis than those without ECE. The predictive usefulness of ECE for salivary gland cancers is unknown, but some extend findings from mucosal head & neck cancer to have included ECE like a high-risk trait in salivary gland cancers. We look into the possibility that ECE is an important prognostic factor in large salivary gland tumours with pathologic node positivity.
Prostate cancer with extraprostatic (extracapsular) extension is a known negative prognostic factor and has clinical and therapeutic implications because it has spread beyond the prostate gland’s fibromuscular pseudocapsule into the surrounding periprostatic soft tissues, most notably the periprostatic fat.
Although ECE has been shown to be a strong prognostic factor in cases of salivary squamous cell carcinoma (SCC), its utility in predicting outcomes in cases of non-squamous salivary cancer is less clear. Patients with high grade, positive surgical margins, or, in the case of salivary SCC, a positive ECE status may benefit from radiotherapy to increase their chances of survival.
