Cervicocranial artery free floating thrombus is an extremely uncommon neurovascular disorder. There is currently no agreed-upon definition of FFT. Because of this, FFT can be confused with intraluminal thrombus or flat mural thrombus.

We describe a middle-aged smoker who presented with several embolic strokes in various posterior circulation areas and responded well to medical treatment for a rare instance of symptomatic FFT inside left vertebral artery spanning V0–V2 segments.
This case study aims to bring attention to the need for a large-scale investigation to determine the best method for managing disorders like FFT inside the vertebral artery by highlighting the difficulties inherent in the clinical approach.
Rarely seen but extremely dangerous, free-floating thrombosis of the carotid artery puts the patient at risk for an immediate ischemic stroke. As soon as the diagnosis of an intraluminal thrombus inside the internal carotid artery is made, prompt medicinal, surgical, or combination therapy is initiated.
